Weaning from mechanical ventilation: stay poised between load and strength.
نویسنده
چکیده
OXYGEN consumption (V̇O2) of respiratory muscles during mechanical ventilation is usually computed as the difference between V̇O2 during controlled ventilation and V̇O2 during spontaneous ventilation. This method, though, is unpractical and sometimes even impossible. In a study published in this issue of ANESTHESIOLOGY, Bellani et al. used another interesting approach to evaluate the value of V̇O2 in weaning. 1

Weaning from ventilator and effect of Blender-Humidifier on outcome of it
Background and objectives: the weaning procedure of mechanical ventilation in many patients is a difficult and long process and increases the time of mechanical ventilation. There are numerous ways to achieve this goal. One common way is using CPAP-ventilator. Considering the lower price of Blender-Humidifier compared to CPAPof ventilator and the limited number of studies in this field, this st...
متن کاملSudden Complete Versus Gradual Weaning from Nasal Continuous Positive Airway Pressure in Preterm Neonates: A Randomized Controlled Trial
Background: Continuous positive airway pressure (CPAP) is used as respiratory support in preterm neonates; however, the best weaning method has not yet been determined. In this study, we compared sudden complete and gradual weaning from nasal CPAP (NCPAP) in preterm newborns. Methods: This randomized controlled trial was conducted on 62 prete...
متن کاملHandgrip Strength Predicts Difficult Weaning But Not Extubation Failure in Mechanically Ventilated Subjects.
BACKGROUND Muscle weakness, defined by the Medical Research Council scale, has been associated with delay in mechanical ventilation weaning. In this study, we evaluated handgrip strength as a prediction tool in weaning outcome. METHODS This was a 1-y prospective study in 2 ICUs in 2 university hospitals. Adult patients who were on mechanical ventilation for at least 48 h and eligible for mech...
متن کاملInspiratory muscle training increases inspiratory muscle strength in patients weaning from mechanical ventilation: a systematic review.
QUESTION Does inspiratory muscle training improve inspiratory muscle strength and endurance, facilitate weaning, improve survival, and reduce the rate of reintubation and tracheostomy in adults receiving mechanical ventilation? DESIGN Systematic review of randomised or quasi-randomised controlled trials. PARTICIPANTS Adults over 16 years of age receiving mechanical ventilation. INTERVENTI...
متن کاملComparison of the effect of pressure support ventilation and volume assured pressure support ventilation on weaning patients off mechanical ventilation after cardiac surgery
Background: Weaning from mechanical ventilation in the intensive care unit is an important step in the treatment process. More patients in less than 6 hours after cardiac surgery can be separated from mechanical ventilation and extubated. However, 20-40% of patients after cardiac surgery, due to dysfunctional ventilator weaning response (DVWR), still remain under mechanical ventilation. Therefo...
